Admissions Requirements

Age Requirements


St. Elizabeth Ann Seton Catholic School requires that a child entering PreK3 must be 3 by September 1st. PreK4 students must be 4 years of age by September 1st. NOTE: Students must be potty-trained. A Kindergarten student must be 5 years of age by September 1st. A child entering first grade must be 6 years of age by September 1st. Age requirements are state mandated for all accredited schools.


Admission Documentation Policy


For all new students, the following documents must be provided to the administration before enrollment is complete:


  1. Admission Application
  2. Emergency Card
  3. Ongoing Directory Release/Permission for Publication Form
  4. Copy of official state birth certificate
  5. Copy of official Baptismal certificate (for Catholics)
  6. Sacramental record of First Reconciliation and First Eucharist, where applicable for Catholics
  7. Final report card copies from previous school(s)
  8. Results of any standardized testing
  9. Signed Transcript Request (release for previous school records for grades 1-8)
  10. Letter of Reference from current school principal or assistant principal (grades 1-8)
  11. Completed Diocese of Fort Worth Health Report (including school physical)
  12. Parishioner Verification Form
  13. Immunization records signed by a physician (no religious exclusions)
  14. Information about special health needs
  15. Documentation regarding psychological, medical or educational evaluations
  16. Registration fee paid in full
  17. Assessment fee paid for all new students entering K through 8th grade

Policy & Updates

Immunization Policy

Every student enrolled in a Catholic School in the State of Texas shall be immunized against vaccine preventable diseases caused by infectious agents in accordance with the immunization schedule adopted by the Texas Department of State Health Services. A student who fails to present the required evidence shall not be accepted for enrollment. The only exception to the foregoing requirement is a medical exemption signed by a licensed physician (M.D. or D.O.) authorized to practice in the State of Texas, including the physicians license number.

Immunizations are not in conflict with the Catholic faith. Conscientious objections or waivers, which may be permissible for attendance in public schools, do not qualify as an exemption in Catholic Schools in Texas. (Atty. Gen. Op. GA-0420)

This policy was adopted by Texas Catholic Conference Education Department, December 2008 and voted on April 3, 2017 to keep as written with no changes.
